Small Claims Court Statement of Claim (Plaintiff's Claim)

A Small Claims Court Action is commenced by filling a Plaintiff's Claim with the Small Claims Court and serving a copy to each defendant.  The claim has to contain the full names and contact information of all the parties, the summary of the facts with reasonable certainty and the amount of money claimed. If the plaintiff's claim is based on a document, a copy of the document must be attached to the claim.
